Welcome to the latest ELS Newsletter.

As always, we have a number of events to inform you about. Two upcoming conferences will have educational input from the ELS: VoiceIstanbul 2024 in April 2024, and the CEORL-HNS congress to be held in Dubin in June 2024.

The world of laryngology has been busy in recent months, including  joint meeting of the Union of European Phoniatricians and the British Laryngological Association – see below for a report on this meeting in London which was held in September.

If you haven’t yet attended one of our Monday Meetings, the next one is on 6 November at 18:00 (CEST) when Cesare Piazza will be talking about Reconstruction of partial/circumferential hypopharyngeal defects by pedicled and free flaps. CEORL-HNS : Let’s go to Dublin in June!


The Confederation of the European Otorhinolaryngology and Head and Neck Surgery was established in 2009 with the aim of creating one voice for ORL-HNS in Europe and beyond. The Confederation biennially organise scientific events with the purpose of facilitating continuing medical education and professional development, and to unify the advancement of ORL-HNS in Europe.

We had a fantastic congress in Milan in 2022 where the ELS was involved in planning and executing a high-level academic program, including instructional sessions for the younger clinicians and challenging debates for the experts. You are now invited to join us in Dublin, Ireland, 15-19 June 2024, for the next biennial CEORL congress:  the laryngological program will be very rich and varied ranging from presbyphonia to artificial intelligence and new technologies, going through glottis stenosis, papillomatosis or thyroplasty, to name just a few. BLA/UEP INAUGURAL JOINT MEETING

The first BLA/UEP Inaugural Joint Meeting held 14 September 2023 in London was a very informative event with high quality international presentations. After the 30th UEP congress in April in Antalya the idea was born to join forces between BLA and UEP. The organising committee has gone above and beyond to provide for amazing keynote speakers and really beneficial, informative and enjoyable talks. The cheese-and-wine reception was a very pleasant way of meeting old friends and new contacts. 

Neurolaryngology was a big topic during the day and a state-of-the art overview of current treatment possibilities was presented. The keynote lecture by prof. Berit Schneider-Stickler on the Neurolaryngology clinic was very comprehensive and informative and many listeners were surprised by the results of functional electrostimulation for unilateral vocal cord palsy and tremor. The choice of the organising committee to invite Neurosurgeons Ludvic Zrinzo & Marie Krueger to talk about functional neurosurgery for disorders of speech was a hit with the audience. Deep brain stimulation (DBS) and focused MRI guided ultra sound treatment (FUS) are already being used to good success for patients with laryngeal dystonia and voice tremor. And prof. Tetsuji Sanuki provided the long wanted answer regarding his type II thyroplasty titanium bridges for adductor type laryngeal dystonia: yes, they will be available in Europe shortly! The ever strong and charming emeritus professor Johan Sundberg had a wonderful remote presentation on phonatory aspects of the voice, and left the audience in awe with his live voice recordings and of course the organ in the background of his room. The in-depth panel discussions on surgical techniques for posterior glottic stenosis and treatment of complex dysphagia were a great addition to the day and the diversity in invited speakers was refreshing.  

We are already looking forward to the next joint BLA/UEP meeting 23-25 May 2024 in Zagreb Croatia and to the biannual Cutting Edge Laryngology Meeting hosted by the BLA which will be held in the first week of October 2024.

EUROPEAN LARYNGOLOGICAL RESEARCH AT ITS BEST,
HOW ELS SUPPORTED MEETINGS CAN LEAD TO INTERESTING DISCOVERIES !

The attitude to recent onset unilateral recurrent paralysis has been the subject of much debate. The debate is still raging.

Where non-ENT surgeons, performing surgeries with a high iatrogenic potential, tend to wait sometimes months for a hypothetical spontaneous recovery, ENT surgeons have adopted a more proactive attitude.

Logopedic treatment is initiated as soon as the diagnosis is made. This initial treatment is supplemented by a proposal for injection laryngoplasty. Here again, there is debate on both sides of the Atlantic as to the timing of this injection.

American colleagues propose injection laryngoplasty from the outset, whereas we Europeans tend to wait beyond 6 months of evolution.

There are several reasons for this continental difference in attitude. Our American friends put forward both empirical-scientific and societal arguments. Among the societal arguments, the virtual absence of remuneration during a work interruption imposes a rapid restoration of the voice, whereas in Europe, compensatory remuneration exists and authorizes a more passive approach. American laryngologists have also developed - with the help of insurers - access to office-based procedures where Europe lags behind. Access to injection laryngoplasty is therefore easier and better reimbursed.

Our US laryngologist friends also put forward the scientific argument that early injection improves the long-term prognosis for voice recovery, and reduces the need for more definitive procedures such as thyroplasty or ansa hypoglossi reinnervation [1].

In Europe too, several authors have described the benefits of injections beyond the lifetime of the substance injected [2].

But how can an injection of a temporary substance bring about a definitive improvement?
During a panel discussion in the shadow of the Swiss Alps at the Winter Days of Laryngology, a research project was launched. The aim was to evaluate possible injection-induced neuroplasticity optimizing laryngeal re-innervation [3].

After injection, two types of sensory feedback stimulating potential neuroplasticity can be described: one auditory, the other proprioceptive.

Indeed, the closure of the glottic leak obtained by the injection instantly improves not only the power of the voice and thus the auditory feedback, but also enables the creation and maintenance of a subglottic pressure modifying laryngeal proprioception.

A European study was able to demonstrate the existence of this proprioceptive neuroplasticity loop in a patient after injection, using functional magnetic imaging. In this individual with recurrent nerve neurapraxia, fMRI demonstrated bilateral activation of the brainstem ambiguous and solitary nuclei immediately after injection. This activation gradually disappears over time.
Further studies will have to clarify the conditions required to obtain this post-injection proprioceptive neuroplasticity.

At present, however, there is sufficient evidence to suggest early injection laryngoplasty to our UVFP patients.

Flexible laryngoscopy doesn’t show any abnormalities and there is normal vocal cord movement. There are some interesting findings on flexible tracheobronchoscopy.

Can you briefly tell me about your career so far?

I grew up in Rouen. At first, I was torn between music and rock climbing before pursuing studies in medicine. I discovered surgery relatively late and was initially impressed by cardiac surgery, then neurosurgery, and finally ENT (Ear, Nose, and Throat) surgery, with Jérôme Andrieu and Danièle Dehesdin. ENT seemed to encompass all the qualities I was looking for: elegant surgery, complex physiopathology, and diversity.

I was fascinated by microsurgery and functional rehabilitation, which became the focus of my practice and research. I first became interested in reconstructive surgery and free flaps, then in otology and skull base surgery. I conducted my medical thesis on selective laryngeal reinnervation and had the opportunity to meet Roger Crumley in Los Angeles and Harvey Tucker in Cleveland (USA).

Captivated by laryngeal transplantation and aiming to enable functional reinnervation, I embarked on my doctoral thesis on the role of the phrenic nerve in this application. The results I obtained allowed me to initiate laryngeal reinnervation in humans as part of a prospective project that began in 2003.

Becoming a Professor at the Rouen University Hospital in 2000, and later the Head of the ENT Department in 2013, I developed laryngology and phoniatrics alongside otology and skull base surgery. As the head of an experimental surgery laboratory and a research team focused on ventilatory and neurological disabilities (UR 3830, GRHVN), I oversee studies on axonal regeneration of peripheral nerves (recurrent nerve, facial nerve, sciatic nerve) in the presence of olfactory glial stem cells and transcutaneous magnetic stimulation. These studies also have applications in horses.

Together with several French laryngologists, we obtained a PHRC (Hospital Clinical Research Program) grant for reinnervated laryngeal transplantation. I played a role in establishing the Rouen Speech Therapy School and strive to instill a passion for research by awarding clinical prizes to interns and surgeons.

Since 2016, I have been organizing a practical course on laryngeal reinnervation, which attracts laryngologists from all five continents. ELS (European Laryngological Society) is an open platform for me, ideal for sharing advancements in the field of laryngology with other clinicians and researchers.


How have you been involved in the ELS in the recent years?

I have participated at all the ELS meeting, presenting my clinical or research results. I am one of the member of the neurolaryngology commission.
I regularly serve as a reviewer in the Archives of Otorhinolaryngology in the field of the laryngology.


You were at the ELS congress in Milan: was there a session that particularly interested you?

I was really impressed by the new development of tracheal reconstruction, but also by the neurolaryngology sessions: neurostimulation and reinnervation have probably a great future.

What are for you the important future perspectives in laryngology?

Treatment of papillomatosis
New techniques of ventilation

Do you have another topic you would like to focus on?

Posterior commissure lesions and rehabilitation. And surgery of oropharyngeal dysphagia.

And to finish can you answer to our laryngologic dilemma : what would be the worst for you : you can’t ever use any type of corticosteroids again ou you can’t ever use a lasers again in your career.

Difficult question! The worst would be to no longer have corticosteroids.
Cold instruments, electrical coagulation, bipolar, microdebrider or radiofrequency can do a lot without laser!
