Welcome to the latest ELS Newsletter.

It was wonderful to see so many colleagues and friends at the ELS congress in Milan in June. With over 500 delegates from more than 60 countries, it was an enormously successful few days. The academic programme was developed by Cesare Piazza, and was extremely diverse and comprehensive. Read the report about the congress published by our media partner for the event, ENT & Audiology News, the link is below.

 INTERVIEW WITH THE NEW ELS PRESIDENT -
PROFESSOR MAŁGORZATA WIERZBICKA

Can you briefly tell me about your career so far?

I am a full Professor of medical sciences. My interests are Head and Neck, with special regard to larynx cancer, but also airways, genetics and basic sciences. I work not only as a medical doctor but as a member of the Polish Academy of Science in topics connected with scc and parotid tumors. I am the chief editor of the HN section in Frontiers in Surgery and Chief Editor of the Polish Journal of Otolaryngology, the organ of Polish Society with 100 years’ history.
 
How have you been involved in the ELS in recent years?


I was a member of the two Scientific Committees (Low Risk HPV and High Risk HPV), a member of the scientific board and since 2023 President of the ELS. My main task will be to work on a PC and organize the congress in 2025 in Warsaw.
 

Do you have any specific projects for the next congress?

Scientific plans will crystallize within a year, but AI, laryngeal cancer and LTS will definitely be the locomotive. I think that a lot of time should be devoted to the rehabilitation of the voice and swallowing. From a practical point of view, Poland is a relatively cheap destination, so it will be possible to invite participants with families, we will be focused on hosting accompanying people and numerous attractions for the youngest will be organized. May family trip!
 
What are important future perspectives in laryngology?

Artificial intelligence. I think that we have to catch the train of deep learning, incorporate the AI tools into the practice and adapt it for our speciality. I think that we are now a bit far away from the basic sciences. I think we can go a step forward not only for molecular diagnostics but as well in perioperative medicine and in the field of immunology in larynx.
 
Can you tell us a place we should visit in Warsaw when we will be there in 2 years?


I think that Warsaw is a modern, cheerful and very young city so please have a look at the old down-town but also at the modern part which became in the last twelve years a European pearl. One year before it was given the status of one of the most pleasant european destinations.
 
And to finish, can you answer our laryngologica dilemma: what would be the worst for you : you can’t ever use any type of corticosteroids again or you can’t ever use lasers again in your career.

The basic tool for me is a laser! I can declare that I will not use corticosteroids if only I could still use the laser. I am Head Neck! But what a tricky question!

The 2024 ELS workshop devoted to the arytenoid cartilage in all its aspects will be held on 25th April 2024 in the framework of the VoiceIstanbul course.

The pre-program in the present state is in attached file. I would like to invite you to submit a presentation, propositions for panels, lectures related any of the topics of the preprogram. From there we will try with the core ELS Presidential council to build an attractive program. We are also welcome to participate in the full voice course organized by Prof. Kürşat Yelken, President of the event. Please send me the tile of your proposition and a short abstract, names and title of the main author and co-authors, e-mails ….

Prof. Yelken paid attention to propose the lower fees as possible to make the course attractive for our all our members. You will find the registration fees HERE.

VoiceIstanbul is an international conference in all aspects of voice which will be held in Istanbul, Turkey between 25th and 28th of April, 2024.

It is a multi-disciplinary meeting designed for present and future voice professionals: otolaryngologists, speech and language therapists, singing teachers and vocal coaches, singers, performing artists and voice scientists. Our aim is to bring together these different specialities, to share knowledge and expertise in all aspects of the voice, and to improve inter-disciplinary collaboration.

Panels, workshops, conferences, case discussion sessions, and posters will be presented to instruct and inform the scientific and artistic communities about advances in voice science, care and training.

VoiceIstanbul is organized by the Voice, Speech and Swallowing Society (Turkey) in collaboration with the Professional Voice Society, the Association of Speech and Language Therapists, and Istanbul Technical University. European Laryngological Society is the invited society for VoiceIstanbul 2024.

We are looking forward to welcoming you to beautiful Istanbul in April 2024.

MONDAY MEETINGS

As usual, the ELS "Monday meetings" continue to take place every first Monday of the month from 18:00 to 18:45 (CEST). These sessions cater to all levels of laryngologists from beginners to advanced practitioners. During the meeting, one of our experts and moderators will guide you through a topic in a practical manner - teaching you the basics and their own approach while answering all your questions in this area.

Scenario: This 65-year-old gentleman with a history of reflux disease presented with a sore throat, dysphonia and progressive shortness of breath over the past year. He's a never smoker. CT showed a mixed density mass in the subglottis with 'popcorn' calcifications and there is no invasion to adjacent soft tissue or bone. Pathology shows granular calcifications in multinucleated chondrocytes. 
 
Questions
1) What is the likely diagnosis?
Answer: low grade chondrosarcoma

2) What treatment would you recommend, and what are your considerations regarding treatment?
Answer: conservative if asymptomatic, endoscopic laser debulking if symptomatic, laryngectomy is reserved for large tumours in which surgery would cause destabilisation of the cricoid.

Dysphonia International Prize winner

Can you briefly tell us about your career so far?

I work as otorhinolaryngologist in the Kaunas Clinic of the Lithuanian University of Health Sciences (LUHS) since 1993. In 2003 I defended my PhD thesis "Importance of the laryngological examination in diagnosing laryngopharyngeal form of gastroesophageal reflux disease". Since 2013 I am a professor of the ENT department of the LUHS. My research fields are: vocal and speech assessment, dysphonia, phonosurgery and atypical forms of GERD.
 
Could you tell us a bit more about your prize winning research regarding “Vocal training could predispose to paediatric dysphonia”? 

Vocal misuse (speaking/singing too loudly, abnormally high pitch) and/or vocal overload (vocal training) are considered the most common causes of pediatric dysphonia. Children with vocal training may be considered as a group at-risk for developing dysphonia. Therefore, we examined n=262 children (age range 6 to 16 years). There were two groups: n=115 from four Music Schools (vocal training - VT group) and n=147 healthy children from four Secondary Schools (vocal untrained - VUT group). For dysphonia screening we used the Lithuanian version of Glottal Function Index (GFI-LT).  Dysphonia was suspected when the GFI-LT score was >3.0 points. Subjects with suspected dysphonia underwent laryngeal examination which consisted of: 1) video laryngostroboscopy – to evaluate possible organic or functional alterations in the larynx and clarify the cause of dysphonia, 2) acoustic voice analysis – to assess voice quality, 3) measure of maximum phonation time – to evaluate the aerodynamics of phonation.

N=28 (10.6%) children from the VT and n=15 (5.7%) from the VUT group did not pass the screening (p<0.05). The mean GFI-LT scores in VT (2.4±3.1) and VUT (1.1±3.1) groups revealed a statistically significant difference (p<0.001). The rate of dysphonia in the VT group (5.2 %) was significantly higher (χ2=25.6, p<0.05) than in the VUT group (2.1 %). Significant correlations were found both between the GFI-LT scores and duration of attendance of Music School (r=0.39, p<0.001) as well as between dysphonia rate and voice load (KLASAK) (r=0.34, p<0.001). We concluded that vocal training could predispose to pediatric dysphonia. The greatest attention should be paid to children who engage in the extensive singing activities. GFI is a valid and reliable test to screen possible voice disorders early.

How have you been involved in the ELS in recent years?

I joined to ELS as member only this year. Many years I attended conferences organized by ELS.
 
What do you think are the important future perspectives in laryngology?

In my opinion, the future of laryngology is to optimise the diagnostics of dysphonia for treatment early as possible, to develop new methods of treatment and to educate people about laryngeal diseases
 
Do you have other topics you focus on?


I'm also interested in laryngeal paralysis, post-tonsillectomy bleeding and substitutional voicing.
 
What have been your highlights of the ELS 2023 conference?

I really appreciated all the presentations about laryngeal stenosis, laryngeal paralysis, laryngeal papillomatosis, and artificial intelligence in ENT. 
 
Here's a clinical dilemma, what would be the worst for you: 'You can't ever use any type of corticosteroids again' or 'You can't ever use a laser again in your career'? 

"You can't ever use a laser again in your career" - would be the worst. I can't imagine laryngology and my work without it.
