Times continue to be very tough for clinicians around the world, but in some parts of the world, Covid infection rates are starting to fall. As vaccinations are rolled out, we all hope that this will make a huge difference to our ability to return to work and leisure activities. In this newsletter, we hear from Prof. Antoine Giovanni (Marseille) about his work during the pandemic.
 
We also hear from our President, Christian Sittel, about the ELS Digital Congress on 28 May. This promises to be a very comprehensive day of discussion and education – scroll down to read more.
 
As this newsletter reaches you, we will just have celebrated World Voice Day – 16 April every year is marked by events celebrating the voice, and you can read more about the World Voice Day groups below.

 VIRTUAL ELS CONGRESS - May 28, 2021

The ELS online congress is now just a few weeks away – on 28 May.  You can register here, and the full programme is here
 
There will be four parallel streams during the day on 28 May, so there will be plenty to choose from!  There will be sessions on oncology, phoniatrics, airway, neuromuscular, HPV, basic science, Covid, and many others.  Some of the highlights of the Congress include:
 
  • Pediatric laryngology. Hasnaa Ismail-Koch is our “liaison officer” and session organizer.  Look out for Zooming into Paediatric Airway management and Paediatric Feeding Issues
  • Neurolaryngology.  We have a number of world-leading clinicians and researchers in these fascinating sessions.
  • Airway. The session on subglottic stenosis will include a number of international speakers from the US, the UK, and the rest of Europe.
 
Ahmed Geneid from Helsinki has put together an excellent panel on Covid-19 in Laryngology, which definitively will be a highlight.
 
Once registered, delegates will be able to access the lectures “on-demand” until August, so it will be possible to watch all of the content in your own time.

Spot diagnosis case 1
 
A 61-year-old male without any history of prior dysphonia presents to you with two months of voice complaints. He is a non-smoker, does not consume alcohol, does not have reflux, and has no known allergies. The endoscopic exam shows bilateral ulcerations with incomplete closure and reduced vibrations on stroboscopic examination. What is your diagnosis?

2nd Edition Surgery of Larynx and Trachea

Dear members,
 
We are now eagerly awaiting the second edition of the textbook “Surgery of the Larynx and Trachea” which should be available this year. Below Marc Remacle explains on how this second edition came about:
“Some ten years ago, Hans Eckel and I co-edited a textbook on surgery of larynx and trachea.
The book dealing with all the aspects of laryngeal and tracheal pathologies, from dysfunction to cancer was well received not only in Europe and all over the world but as the majority of authors were European, a European flavor was predominant!
But of course, after ten years, many chapters are outdated, and new approaches are missing.
Springer, publisher of the first edition, gave us the opportunity to edit a 2nd edition
Again, the majority of authors will be not only from Europe but will also be ELS members.
And so, this time the flavor will be an ELS one.
The Presidential as well as the Scientific councils of the ELS have proudly accepted that the book has been edited under the umbrella of our society.
Non-European colleagues from USA, Middle- East and Asia, members of ELS accepted to join us in this effort.
New chapters like surgery through transnasal flexible videoscope or for modification of the voice pitch have been added, and others have been rewritten to match the recent advancements in our specialty, resulting in a total of 35 up-to-date chapters.
We hope that same as for the first edition, this textbook will be well received and will contribute to the teaching of our juniors as well as to the knowledge of the seasoned laryngologists”.

Interview with a laryngologist about the impact of the Covid-19 situation in their daily practice

During the Covid lockdowns, in the ENT department of Marseille University Hospital, we were not requisitioned in Covid units and we neither performed tracheotomies in intensive care units.

However, our activity has been strongly impacted by the Covid epidemic, in particular our surgery program due to a decrease of available operating rooms. Patients were categorized into three types:
  • Category A patients requiring urgent surgical management (for example in the laryngology field, a transorale laser cordectomy)
  • Category B patients requiring semi-urgent care (for example, leukoplakia of the vocal folds or patients with laryngeal paralysis with swallowing disorders)
  • Category C patients who did not require urgent care (for example benign lesions of the vocal folds such as vocal nodules)
The total number of hospital beds excluding covid patients was also reduced. These disruptions have resulted in delayed treatment for some patients and organizational difficulties, but they also allowed two important advances in our clinical practice.

On one hand, we have been able to develop office-based video-endoscopic laryngeal surgery. Indeed, being done under local anesthesia and directly at office, we could continue this activity without congesting the precious operating rooms (while obviously taking important precautions: adequate personal protective equipment and preoperative PCR test). We were therefore able to continue the laryngological activity by intensifying this practice, which allowed us to take care of patients who could not have been treated otherwise during this period. This has also enabled us to enrich our practices in this technique and to carry out new treatment strategies such as laser cauterization of laryngeal papillomatosis or Reinke's edema, injection of corticosteroids into vocal nodules, etc.

In addition, we have developed a prototype mask allowing safe endoscopies through the placement of a perforation located opposite the nasal cavity and  kept impervious by a plastic valve and a folding sticker. We then teamed up with a company for the development and marketing of this mask that we hope to be quickly accessible to all ENTs in France and Europe.

This period of confinement also led us to develop our translational research activity, in particular about the link between voice, singing and the projection of potentially infectious droplets. Indeed, particles emitted when speaking are one of the main modes of transmission of the Covid 19. We aim to study thoroughly the production of particles emitted when speaking, to analyze the velocity of particles and perform numerical simulations of their trajectories. This knowledge is a crucial element of prevention. Finally we will study how wearing a mask degrades speech intelligibility, by comparing masks with different geometries, and how the emission of particles correlates with the actual degree of voice load.

Some positive elements have thus been able to emerge among the misfortunes linked to this covid epidemic.​

World Voice Day – 16 April 2021
Back in 2002, a group of voice clinicians (physicians, speech-language pathologists and singing teachers) launched an annual event, with the aim of increasing public awareness of the importance of the voice and alertness to voice problems.
 
The mission of the group is to share the excitement of voice science, pedagogy and the vocal arts as an application of all the above mentioned areas with the public and with funding bodies by organizing a global celebration of the World Voice Day on April 16 each year, joining forces with existing groups that have the same goal. More specifically, the group will
  • organise a global choral concert, internationally broadcasted in real time, going from country to country, starting in the far East and ending in the far West.
  • arrange a global series of talks, internationally broadcasted in real time, as well as videotaped for internet distribution, about the many different facets of the voice.
  • identify and ask one person, a “pivot”, in as many countries as possible, who assumes the responsibility to initiate and coordinate various events in that country, on the World Voice Day
  • create a web page where all events are listed and that also contains information about voice and voice science, and interactive voice analysis programs.
