Welcome to the October edition of the ELS Newsletter.  It is wonderful to see so many meetings happening “in person” as we emerge from the pandemic.  See below for a number of exciting events happening in the coming months.  Our “Monday meetings” are proving very popular – scroll down to see a report on the meetings so far, and to hear about the events we have planned. 6th EUROPEAN LIVE SURGERY BROADCAST -
November 24, 2021

You are cordially invited to the 6th European Laryngological Live Surgery Broadcast, which will be held on 24th November 2021 online.  The day will be filled with different laryngological surgeries including laryngeal cancer, benign pathology of the larynx, neck dissection, phonosurgery and others.

11 European laryngological departments, more than 20 operations, comments from the moderators, discussion and questions from the viewers – all will be available during the broadcast at no cost.

Interview with Prof. Szyfter about the organisation and success of 6th European Live Surgery Broadcast
  • Please tell us about how the ELS live surgery broadcast project started.

    Between 2010 and 2014 at the ENT department of the University of Poznan, we had organised 8 live surgery broadcasts from operating theatres – this used supercomputing techniques, and worked very well.  Subsequently, at a 
    2014 meeting of the ELS Board (together with Poznan ENT department) we came to an agreement to start the ELS live surgery broadcast, in collaboration with 10 other European clinics. 

     
  • Can you please describe the project?

    The 6th European Laryngological Live Surgery Broadcast will happen on 24th November 2021.  All details on our event are on the website http://els.livesurgery.net/.
    The organisers are the European Laryngological Society, the University ENT Department in Poznan, the Poznan Supercomputing Centre, and the Mayor of Poznan. The co-organizers are Olympus, Storz, Zeiss, Lumenis, Cochlear and Xion.
    11 departments from Europe, 5 moderators, and many IT engineers are responsible for the organization.  We will be featuring than 20 surgeries done in 11 ENT departments across Europe. We expect about 30-40 thousand viewers.
     
  • What are the goals of the project?

    We aim to provide education of the young laryngologists all over the world.  In particular, there will be an opportunity to see new types of laryngological surgeries.  We hope to spread surgical modern ideas to all continents.
    It is particularly useful that there can be live discussion between surgeons and moderators.  And it is important to not the participation in this broadcast is free of charge!

     
  • Who is involved? 

    10-12 European ENT departments from Hamburg, London, Essen, Barcelona (2 centres), Luxembourg, Marseille, Genoa, Cagliari, Lubljana and Poznan.
    4 -5 moderators in the studio in Poznan: Prof. F Dikkers, Prof. Klussman, Prof. Piazza,  Prof. Bernal-Sprekelsen and Prof. Szyfter.

     
  • How often do the broadcasts happen?

    Once per year in November-December.
     
  • Please tell us about the future.

    Our aim is to maintain good quality of the surgeries, and to optimise the quality of the sound and image.  As always, we want to include new good laryngology departments from Europe. Perhaps future broadcasts will be in 3D!

MONDAY MEETINGS

Dear Members,

The special ELS educational programme - the so-called "Monday meetings" - cater to all levels of laryngologists from beginners to advanced practitioners. The Monday meetings take place every first Monday of the month from 18:00 to 18:45 (CEST) and are aimed at residents and fellows - although of course, everyone is welcome! During the meeting, one of our experts and moderators will guide you through a topic in a practical and interactive manner - teaching you the basics and their own approach while answering all your questions in this area.

Last Monday Meeting held by Fred Dikkers on 4th October with the topic of  Recurrent respiratory papillomatosis - Introduction was a huge success as the audience number grew larger, and we hope that the popularity of the Monday meetings will be steadily increasing as the knowledge of this educational programme spreads around. The audience was from many different countries and went well over 30. You can watch the recording of the session by clicking on the recording link below.

Don't miss our next meeting, which will take place on 1st November at 6:00 pm CEST - Ricard Simo will be talking about Management early laryngeal cancer, which will be followed by Management of advanced laryngeal cancer on 6th December.

The upcoming year will be filled with a variety of topics covering Laryngopharyngeal Reflux Disease, Glottoplasty in male-to-female transgender patients and Laryngeal EMG, just to name a few. We will keep you informed.

Interview with Prof. Isabel Vilaseca about the preparations of the CEORL-HNS 2022 in Milano

  • Please let us know more about the evolution and preparatives of the upcoming CEORL-HNS congress in Milano 2022? 

    After a period of pandemic that forced to postpone the 6th Congress of the CEORL-HNS for a year and in view of the positive evolution of the situation, we are looking forward to Milano 2022 to become the first major face to face congress bringing together all subspecialties in Europe. 

    At this point, we have already completed the contents of the round tables, keynote lectures and instructional courses. To create the program, we have relied on the recommendations of the subspecialties and a wide range of international experts who have agreed to participate in the event. 

    Medical companies are also ready and eager to present their new products and to demonstrate the latest in medical technology.

     
  • Can you tell us the main features that will be discuss in the meeting?

    With sixteen rooms running synchronously, there will be enough space to deepen and debate on the novel diagnostic aspects and therapeutics of the specialty. 

    The congress will include the latest developments in the fields of otology, rhinology and laryngology but also in pediatrics, head and neck cancer, salivary gland disorders, voice, dysphagia and sleep disorders. Special emphasis will also be placed on the training of young people, repeating the experience of the basic track for residents initiated in Brussels, which was so successful.  This time we have included the youngest in its design.

    There will be also space enough for all those who wish to participate with abstracts and contribute with their experience to increase knowledge in our specialty.

     
  • It is impossible not to talk about the pandemic. The board needs to prepare COVID-19 arrangements for the meeting? 

    Based on expert opinion, we assume that the pandemic situation will probably be stabilized in Europe within a year. As preparations progress, the protocols to be implemented to minimize the risk of contagion for all attendees will be defined.
     
  • Regarding Laryngology, can you mention the topics of the key-lectures?

    Within the field of laryngology, special emphasis will be placed on laryngotracheal stenosis, which has seen its incidence increase with the pandemic. Topics such as laryngeal paralysis and the future role of laryngeal pacing, technical refinements of Wendler glottoplasty, neuromodulation in spasmodic dysphonia or robotic surgery for supraglottic tumors will be specifically covered in the program. 

    The program also will include the latest knowledge on exudative lesions of the vocal cord, management of vocal scars, laryngeal papillomatosis, office based procedures or an update on bioendoscopy or laryngeal electromyography. 

     
  • Any final remark?

    Apart from the scientific sessions, we are also working on a fantastic social program. There is no doubt that Milano with its culinary tradition and its friendly and elegant atmosphere is the ideal place for this. 

    So I would like to encourage all the ENT specialist to lose their fear and participate in the event, so that we can make it one of those congresses that will remain in our memories forever.  After two very hard years, we certainly deserve it.

OBSERVERSHIP

ELS presents a new branch of the educational program for its members - observerships and mini-observerships!

One of the main objectives of the ELS is to promote and foster the professional skills of members. Therefore we actively maintain a network of experienced clinicians who are enthusiastic about welcoming visitors and sharing their expertise. In these observerships, the aim is to get acquainted with a technique or an area of laryngology, watch the clinician in their work, pick up tips and tricks for your own practice, and pose any questions you might have. During an observership, the guest will not actively perform procedures but rather join and observe the laryngologist in her or his practice.

The duration of the observership can vary with so-called “mini-observerships” consisting of just a couple of days to a week.

Dear members, colleagues and friends,

One of the most common diagnostic procedures that ENT doctors are performing globally is the fiberoptic endoscopic evaluation of swallowing (FEES).

Due to the increase in aged patients this diagnostic procedure is becoming more and more requested by patients and different stakeholders - and the support of an additional tool was one of the most desired requests from scientifically working dysphagia specialists. 

In the last few years Susanne Fleischer, MD and Markus Hess, MD, from the Medical Voice Center (MEVOC) in Hamburg used the NBI (Narrow Band Imaging - Link to the website) as an assessment routine in clinical dysphagia diagnostics to support them during their flexible endoscopic procedure in the office.

NBI, Narrow Band Imaging, proprietary technology of Olympus Corporation, is an optical filtering technique that was used in the last years mainly for supporting ENT doctors to better visualize the mucosal vascularization patterns and support healthcare practitioners to improve the diagnostic outcome (references below).
Now we know that in FEES examinations the use of NBI in combination with a food colorant is supporting the doctor significantly to (a) detect and (b) enhance a penetrated and/or aspirated bolus with an enormous high color contrast.

As early as in 2016 Susanne Fleischer and Markus Hess published an article ‘High-sensitivity FEES with NBI-illumination’ in The Laryngoscope (https://onlinelibrary.wiley.com/doi/10.1002/lary.26328) explaining how the use of NBI and green food colorant can be used to improve clinical diagnosis in cases of questionable penetration and aspiration during swallowing.

A detailed explanation on how to use NBI for FEES is outlined in a tutorial video released by Dr. Fleischer and the Medical Voice Center in Hamburg.
