As many countries push ahead with Covid vaccination programmes, we can begin to hope that in-person meetings will be going ahead in the coming months.  But in the meantime, we have all been making the most of online gatherings, and it was wonderful to see so many of you (400 participants!) at the ELS online congress a few weeks ago.  Christian Sittel put together a fantastic programme, which is still available online – see his message below. You might also want to check out our new Monday evening meetings, in which we will be covering a different topic each month.

Interview with Cesare Piazza as new ELS President about his goals for the society
  • Tell us about your place of work and your clinical practice

    Since September 1996, when I started as a Resident, I work at the Spedali Civili, University of Brescia, one of the largest Italian hospital, recruiting patients from all over the country. Except for a 3 year and half parenthesis as Chief of the Head and Neck Unit of the National Cancer Institute of Milan, my entire career has been spent within the University of Brescia, of which I became Chief of the Department of Otorhinolaryngology – Head and Neck Surgery and Full Professor in September 2020. My clinical and research practices of the last 25 years have been always based on 3 pillars: head and neck cancer, airway, and microvascular reconstructive surgery.
     
  • How long have you been involved with the ELS?

    My first experience within the ELS dates back to September 1998, when I attended to the Second Meeting of the Society, in Rome. As a second year Resident, I had the honor to present a couple of free papers and a young Hans Eckel is the first moderator I remember so kind with me to make questions with the intent of clarifying some concepts without (further) embarrassing the speaker… Two years later I participated to the meeting in Paris and I became Member of the Society. I never missed one meeting since then and I really saw the ELS becoming what it is now: a reference in the field, an open space in which discussing the most recent nuances in laryngology, while the youngest may learn the basics in a friendly environment.
     
  • What are the highlights of the next 24 months for the ELS?

    As President of the Society, I would like to improve the following aspects: 1) disseminate basic and more advanced knowledge in the different fields of laryngology through 1-hour online lectures, webinars, and in presence brain storming involving the protagonists of our specialty as well as those of other fields, including informatics, engineers, and biologists; 2) implement the number of position papers, consensus, guidelines and reviews produced by our Scientific Committees and published on the Eur Arch Otorhinolaryngol; 3) promote clinical trials, prospective multicentric studies, and translational activities with industries and sponsors involved in new technologies and drugs. I am sure it is a quite ambitious design, but I know the ELS, and I know it well!

MONDAY MEETINGS

Dear Members,

The ELS is starting a new educational program that will cater to all levels of laryngologists from beginners to advanced practitioners. The first series to start in this program is the so-called "Monday Meeting". The Monday meetings will take place every first Monday of the month from 18:00 to 18:45 (CEST) and are aimed at residents and fellows - although of course, everyone is welcome! 

During the meeting, one of our experts and moderators will guide you through a topic in a practical and interactive manner - teaching you the basics and their own approach while answering all your questions in this area.

Don't miss the second meeting, which will take place on 2nd August at 6:00 pm CEST - Malgorzata Wierzbicka and Joanna Jackowska will be talking about Iatrogenic laryngotracheal stenosis

Spot diagnosis case 2
 
A 40-year old female patient with voice complaints since 6 months underwent a diagnostic microlaryngoscopy. During the procedure, a firm swelling of the false vocal fold and the Morgani sinus was seen on the right side with a smaller, similar deposit in the left Morgagni Sinus. After removing the false vocal fold a soft, diffuse, granular yellow substance was found underlying the mucosa extending onto the thyroid cartilage and over to the contralateral vocal fold. What is the diagnosis?

The first round of the competition took place in the previous Newsletter. Answer to spot diagnosis case 1: idiopathic ulcerative laryngitis

Idiopathic ulcerative laryngitis is a condition associated with prolonged dysphonia and bilateral ulcera of the vocal folds without responding to protonpump inhibitors, antibiotics or antifungal therapy. Often preceded by an upper airway infection and resolution of the condition will be spontaneous although it can take months. The cause is unknown. The typical clinical appearance can lead the physician to choose a path of watchful waiting, withholding aggressive treatment or biopsies of the vocal folds.

Interview with a laryngologist about the impact of the Covid-19 situation in their daily practice

Dr. Isabel Garcia-Lopez - La Paz University Hospital, Madrid, Spain

  • What has happened to your outpatient clinics during the lockdowns?
During the first wave of the pandemia (March and April 2020), due to the huge number of covid patients, the otolaryngology department of my hospital was “closed”. All of us were enrolled in “100% COVID activity”. Therefore, my outpatient clinics were closed. What we did was to revise every day the charts of the appointed patients and call them one by one in order to confirm that the appointment may be postponed.
From May 2020 onwards, outpatient clinics have been opened and I have been seeing patients normally.
  • Have you been able to continue with "routine" operating, or just urgent/cancer cases?
During these “terrible two months” (March and April 2020) we canceled all our surgeries. The urgent and cancer cases were referred to a specific center in the city that maintained surgical activity. From May 2020, we have recovered our routine surgery, both oncologic and non-oncologic cases.
  • Have you been doing tracheostomies? Open or percutaneous?
I have been doing tracheostomies during the pandemia. In my case (and in all otolaryngology departments) we have always done open tracheostomies.
  • Have you been doing other jobs? For example, proning patients on ITU? Working on other wards?
In our department, we were very well organized in groups doing the different tasks we were asked for: emergency department, COVID admitted patients… In my case, my job was to go every day to the ICU and do the tracheostomies that were necessary.
 
OBSERVERSHIP

ELS presents a new branch of the educational program for its members - observerships and mini-observerships!

One of the main objectives of the ELS is to promote and foster the professional skills of members. Therefore we actively maintain a network of experienced clinicians who are enthusiastic about welcoming visitors and sharing their expertise. In these observerships, the aim is to get acquainted with a technique or an area of laryngology, watch the clinician in their work, pick up tips and tricks for your own practice, and pose any questions you might have. During an observership, the guest will not actively perform procedures but rather join and observe the laryngologist in her or his practice.

The duration of the observership can vary with so-called “mini-observerships” consisting of just a couple of days to a week.
